**Outreach Specialist Position Summary The Outreach Specialist is responsible for the coordination of Veteran Outreach, Presentations and Community Linkages for a specific geographic area. This must be available to assist in accessing Veteran Benefits and Resources. The Outreach Specialist in the Community Integration Program works as part of a multi-disciplinary team that serves clients in the community, providing a variety of peer support, advocacy, and recovery services to veterans transitioning to community living, which would include, but not be limited to, the following activities: Essential Functions Pre-Screening Veterans during outreach activities and conduct presentations to Community Based Organizations serving the Veteran population. Develop and expand community linkages (Internal and External) to meet the needs of the Veterans and their families in the system, while documenting progress. Assist with development of referral sources for accessing various supportive services within their local Communities i.e., Food banks, Discount and/or donated furniture etc., and Libraries. Assists clients in developing community living skills and utilizing community resources by discussing common transitional experiences. Assists clients in developing independent living skills such as housekeeping, cooking, shopping, budgeting, using public transportation, and attending to personal grooming, etc. through demonstration and coaching. Assists clients with housing needs by facilitating access to supportive or permanent housing. Conveys community & client cultural patterns and attitudes to multi-disciplinary team. Interacts with clients to assist in identifying independent living issues and providing referrals to community resources and potential financial resources. Assists clients with obtaining and completing the appropriate application forms for various resources. May transport/accompany clients to appointments and self-help activities as needed Assists with screening and eligibility determination Attend regular staff meetings and trainings when assigned by the Program Manager Adopts an whatever-it-takes approach in assisting clients to retain housing and independent living. Other duties as assigned Working ConditionsMost of the direct service work is performed in the community. Thus, the Outreach Specialist must have the ability to walk, stand, bend, squat, climb, kneel and twist on an intermittent or sometimes continuous basis, the ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead; ability to operate computer, fax and telephone; and the ability to lift up to 25 lbs. Requirements Minimum Qualifications Veteran status strongly preferred At least 2 years experience working with people who have mental illness and/or substance use issues. Must obtain First Aid/CPR certification in first 90 days of employment Must pass health screening in first 90 days of employment Must receive TB testing once yearly A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions Skills, Knowledge & Abilities Commitment to the holistic mission of The Salvation Army and the Haven Experience working with persons who are homeless and/or disabled If in recovery, at least 1 year of being clean and sober to the present, with evidence of working a program of personal recovery Commitment to working with disabled veterans Must have appropriate written and verbal communication skills Ability to work well with others and committed to the vision that the Haven be a program of excellence* The salary listed in the header is an estimate based on salary data for similar jobs in the same area.
