Position Summary:

The Caregiver Coach works directly with family caregivers and their care recipients through a combination of empathy, technology, and proven methods of care. This individual provides dedicated support, education, and guidance to caregivers, giving them tools and confidence to enable better care for loved ones at home. Support and resources are provided via telephony and proprietary digital chat technology, using evidence based methods of care that address both health care and social needs. This position is based in Quincy, MA.

What You Will Do:

* Engage, motivate, coach, and educate caregivers via phone and digital technology
* Empower caregivers to manage their own psychosocial, physical and environmental health to improve and maintain wellbeing
* Research opportunities for external supports and services for referral purposes
* Assess caregiver and care recipient needs; assist the caregiver in development and achievement of goals
* Collaborate with key stakeholders to assess coaching plans and caregiver engagement; identify challenges and solutions
* Document and report out on coaching activity with a focus on quality and outcomes; make recommendations as appropriate
* Solicit feedback from caregivers to identify opportunities for additional support and process improvement

What You Will Bring:

* Experience working with elderly, disabled and/or dementia population a plus
* Bachelor's degree in a related discipline (Social Work, Behavioral Health, Dementia Care), or equivalent work experience
* Exceptional active listening and communication skills with strong empathy and excellent decision-making
* Proven experience accessing and locating community resources
* Comfort using (and teaching others to use) digital platforms and mobile apps including text based systems
* Ability to multitask in a multi system environment; proficiency with Microsoft Office required; case management experience a plus
* Multilingual a plus
